Перелом дистального метаэпифиза лучевой кости — самый частый перелом у человека. Большинство авторов рекомендует оперативное лечение при оскольчатом внутрисуставном переломе. Предложено большое количество различных методов оперативного лечения, тем не менее, на настоящий момент статистически не выявлено значительного превосходства какого-то одного метода над другими в долгосрочной перспективе наблюдения. При оскольчатом переломе дистального метаэпифиза лучевой кости предложено использование спицевого аппарата внешней фиксации в качестве этапного лечения (первичной иммобилизации) на период от травмы до окончательного остеосинтеза волярной пластиной с угловой стабильностью винтов.
Целью исследования являлось определение эффективности и целесообразности использования аппарата наружной фиксации на этапе первичной иммобилизации оскольчатых внутрисуставных переломов дистального метаэпифиза лучевой кости перед остеосинтезом пластиной на следующем этапе лечения.
В задачами исследования входило проведение сравнительного анализа ранних и отдаленных результатов оперативного хирургического лечения – остеосинтеза пластиной у пациентов с оскольчатыми внутрисуставными переломами дистального метаэпифиза лучевой кости при использовании аппарата наружной фиксации в качестве первичной иммобилизации и при применении закрытой репозиции с последующей предоперационной иммобилизацией классической тыльной гипсовой лонгетной повязкой.
Пациенты и методы. Исследование построено на анализе результатов хирургического лечения 81 пациента с оскольчатым переломом дистального метаэпифиза лучевой кости. Данные переломы считаются нестабильными и консервативный метод лечения не является предпочтительным. Пациенты были разделены на 2 группы по 41 и 40 человек. У пациентов первой группы (группа контроля) на первом этапе была применена закрытая репозиция с последующей предоперационной иммобилизацией классической тыльной гипсовой лонгетной повязкой. Пациентам второй группы (группа исследования) сразу при поступлении в качестве первого этапа лечения выполнена внеочаговая фиксация лучезапястного сустава с его дистракцией в аппарате наружной фиксации (АНФ) без попыток закрытой ручной репозиции. Через несколько дней после уменьшения отека, пациентам обеих групп выполнялась открытая репозиция с остеосинтезом лучевой кости ладонной пластиной с угловой стабильностью винтов. Для оценки функции верхней конечности в послеоперационном периоде использовали опросник DASH. Результаты оценивали через 3,6,12 и 18 месяцев.
Результаты и обсуждение. В раннем послеоперационном периоде, через 3 месяца после операции с применением спицевого АНФ в качестве этапного метода лечения выявлено достоверное улучшение функции верхней конечности (оцененной по DASH). Через 1,5 года после оперативного лечения статистически достоверной разницы в результате лечения выявлено не было. Мы предполагаем, что дистракция в кистевом суставе с применением АНФ ускоряет уменьшение отека в предоперационном периоде и облегчает последующую репозицию при остеосинтезе лучевой кости.</abstract><trans-abstract xml:lang="en">Introduction. Distal radius fracture is the most frequent fracture in humans. Most authors recommend surgical treatment for intraarticular fracture. A large number of different methods of surgical treatment have been proposed. However, in the long-term treatment no significant superiority of any one method over others has been statistically revealed.
Objectives. To study the results of using of Ilizarov external fixation as primary fixation (first step treatment) for the period from injury to final ORIF with a volar locked plate.
Patients and methods. The study is based on the analysis of the results of surgical treatment of 81 patients with multifragmentary intraarticular distal radius fractures. These fractures are considered unstable and conservative treatment is not preferred. Patients were divided into 2 groups of 41 and 40 people. In patients of the first group (control group), at the first stage, a closed reduction was applied followed by preoperative immobilization with a classical cast bandage. Patients of the second group (study group) as the first stage of treatment performed fixation of the wrist joint with its distraction in the bridge Ilisarov external fixator. In a few days after reducing swelling all patient had ORIF with volar locked plate. To assess the function of the upper limb in the postoperative period, a DASH questionnaire was used. The results were evaluated at 3,6,12 and 18 months.
Results and discussion. In 3 months after surgery revealed a significant improvement in the function of the upper limb (estimated by DASH). After 1.5 years after surgical treatment, no statistically significant difference in the result of treatment was revealed.
